Research completed

1.         A single centre, feasibility study to evaluate clinical utility of limited transthoracic echocardiography in guiding intraoperative haemodynamic and fluid management in renal transplant recipients.

This observational study was conducted in end stage renal disease (ESRD) adult patients scheduled to undergo renal transplant surgery under general anaesthesia assessed a) whether it is feasible to measure the changes in left ventricular dimensions by transthoracic echocardiography (TTE) as intravenous fluids are administered during the renal transplant surgery and b) to determine the concordance of these echocardiographic variables with data obtained from the central venous pressure trace and systolic pressure.  Out of 153 readings, 112 readings (73.2%) showed concordance regarding volume status between echocardiographic variables and data obtained from the central venous pressure and systolic pressure variation. However, 41 readings (26.7%) failed to show such relation regarding volume status.2D TTE is a feasible, non-invasive option for guiding volume management in ESRD patients undergoing transplant surgery and shows fair concordance with data obtained from invasive monitoring.

2.         Impact of single dose intravenous tranexamic acid on peri-operative blood transfusion requirements in burn patients: a prospective, randomised trial.

Preoperative administration of a single dose of tranexamic acid significantly reduces blood loss during debridement of burn wounds and skin graft harvesting surgeries without increasing the risk of untoward side-effects or complications.

  1. Preemptive multimodal Analgesic Regimen reduces Postoperative Epidural Demand Boluses in Traumatic Femur Fracture Patients

Use of preemptive multimodal analgesic regimen reduced the requirement of demand epidural bolus in patients undergoing fixation for shaft of femur. Significant reduction in mean number of epidural demand boluses was observed in preemptive multimodal group (3.09 (1.24) compared to placebo group (5.59 (2.17); p=0.00. Time (hrs) to first rescue epidural bolus was also significantly greater in preemptive multimodal group 4.75 (3.09) than placebo group 3.80 (1.93) p=0.04.

4.         Effect of pre operative anxiety on postoperative recovery and pain scores in patients of infertility undergoing hysterolaparoscopy under general anaesthesia

The objective was to determine if high levels of anxiety in the preoperative period affects postoperative recovery and pain in patients with infertility undergoing hysterolaparoscopy under general anaesthesia. A total of sixty female patients (age 18-40 years) diagnosed with primary infertility undergoing hysterolaparoscopy under general anaesthesia were divided into low or high anxiety group based on their Amsterdam pre operative anxiety and information scale (APAIS) score. Thirty two patients reported APAIS more than 13 (high anxiety). These patients had longer time to accept oral intake (4.84±1.14 hours) and higher incidence of PONV as compared to 28 patients reporting APAIS less than 13 (low anxiety). Also, a greater number of patients in this group required rescue analgesic at 2, 4, 8 and 24 hours postoperatively. All these findings were statistically significant (p<0.001).  We concluded that high levels of preoperative anxiety in patients with infertility affects the postoperative recovery anaesthesia and pain scores in patients undergoing hysterolaparoscopy.
